A travel nurse's first week in Cleveland

Most of what is written for travel nurses could be about any city. Ohio is affordable. Ask about your stipend. Pack layers.

This is about Cleveland specifically, in the order the week actually happens.

If you hold an unencumbered multistate license from another compact state, you do nothing with the Ohio Board of Nursing before your first shift. Ohio joined the Nurse Licensure Compact on January 1, 2023, and the board does not register or license you at all. Verification is your employer's job. The things that will actually cost you time are parking registration, the overnight gap in the Red Line, and a fare rule almost nobody arriving knows about.

Your license, in two cases

Ohio's employer FAQ from the Board of Nursing is unusually direct about this:

As long as the nurse's MSL license from another Compact State is unencumbered per the Nursys.Com report, that nurse can work in Ohio immediately as of January 1, 2023.

And:

Nurses with an MSL from another Compact State will not be registered or licensed by the OBN, it is the employers' responsibility to ensure that the nurses' multistate licenses are current and unencumbered.

So if your home state is in the compact and your license is clean, there is no Ohio application, no Ohio fee, and no waiting period. Your agency may still ask for documentation. The state does not.

If your home state is not in the compact, you need an Ohio license by endorsement. The board publishes one useful bridge: reciprocity applicants may be issued a temporary permit to practice in Ohio, valid for 180 days, while the rest of the application is completed. Fees are $75 for a single-state license and $100 for a multistate one, and every applicant submits fingerprints for state and federal background checks.

If Ohio becomes your primary residence, which happens to people who keep extending, the National Council of State Boards of Nursing rule is 60 days: you apply for a license in your new home state within 60 days of moving. Proof of Ohio residency is a voter registration card, a tax return, a W-2 or a military form with an Ohio address.

One thing nobody publishes: the Ohio Board of Nursing gives no processing time for any application type. We looked across eleven of its pages. Every number you find online for this comes from a staffing agency rather than the board.

The badge, and the parking rule that catches people out

Cleveland Clinic's main campus Parking Directive is a real, current document, last approved February 11, 2026. It covers contract staff explicitly, and the first line is the one to act on:

All Cleveland Clinic badged employees and non-employees must register with Parking Services for an assignment regardless of shift, duration or frequency to campus

Registration is not optional for a 13-week assignment, and the process for contract staff is the same as for employees. Non-employees are badged with a colored background rather than white.

Two rules follow from that, and both cost money if you learn them the hard way.

You cannot park in patient or visitor parking. The Directive allows it only when you are on campus for your own appointment, at posted visitor rates.

There is an after-hours access window, and it is written in a way that fits a night shift and nothing else.

Weekday entry after 6:00pm and before 5:00am the following day, WITH exit after 8:00pm and before 10:30am the following day (Mon-Fri)

Weekend entry after 6:00pm Friday and before 5:00am Monday WITH exit after 8:00pm Friday and before 10:30am Monday

Line that up against a 7 p.m. to 7:30 a.m. shift and it fits exactly. Line it up against a 3 p.m. start and it does not, which is where people get caught. Arriving before 6 p.m. and taking a ticket instead of badging in is a named violation, carrying $10 per day and possible tire immobilization with a $50 removal fee.

The payment rule is different for you than for staff. Employees are payroll-deducted. Badged non-employees pre-pay by the fifth working day of the month, and the Directive is blunt about what happens otherwise: failure results in assignment and access termination.

Garage entry is a badge tap. A windshield transponder is available for a $15 fee.

We are not publishing current staff parking rates, because Cleveland Clinic does not. The Directive says the fee table lives on an internal intranet page. The only rate sheet on the public web is marked February 2021, five years older than the Directive that governs it. Getting to a shift without a car

Two RTA lines matter, and they behave very differently at night.

The HealthLine runs all night. RTA describes it as operating 24 hours a day, seven days a week, and the published timetable backs that up: there is no gap. What the timetable also shows is that the overnight headway is 30 minutes, not the 10 to 15 minutes advertised for peak. Plan for a half-hour wait at 2 a.m.

The Red Line does not run all night. It runs every 15 minutes, all day, with the same timetable on weekdays, Saturdays and Sundays. Then it stops. At the E 105 – Quincy station, which is the Cleveland Clinic stop, RTA's timetable shows:

What that means in practice: finish at 7:30 a.m. and you have both options. Finish at 1:30 a.m. and the HealthLine is running every 30 minutes, but the last Red Line train has gone and the next one is around three hours away.

The fare rule worth knowing before you buy a pass

RTA fares are $2.50 for a single ride, $5 for a day, $25 for seven days and $95 for a month. Most people arriving on a 13-week assignment buy the monthly pass.

You may not need to. RTA runs fare capping:

Fare capping ensures you always get the best deal. It gives you the savings of a daily, 7-day, or monthly pass, without the stress of paying for one upfront.

Tap or scan each ride, and once your spending reaches the price of a pass, the rest of that period is free. The caps match the pass prices: $5 daily, $25 for seven days, $95 monthly. Daily caps reset at 3 a.m. Monthly caps reset on the first of the calendar month.

Two conditions. Cash paid into a farebox does not count toward the cap, and neither do pre-purchased passes, so you need the card or the app. And since May 7, 2026, mobile ticket holders have to scan the validator when boarding. Showing a phone screen is no longer accepted.

Food on a three-by-twelve

Three twelve-hour shifts a week produces a particular problem: four days when you can cook, three days when you will not.

The useful fact is that one full grocery store is within walking distance of main campus. Meijer Fairfax Market, at 2190 E. 105th Street, is open daily from 6 a.m. to 10 p.m., which covers a day shift on either end and most of an evening. Everything else needs a car.

The stipend, and what the federal numbers actually are

Two sets of published figures come up constantly in travel nursing, and both are routinely described inaccurately. Here is what each one is.

The GSA per diem is what the federal government reimburses its own employees for lodging and meals. For Cleveland and Cuyahoga County:

Both are flat across all twelve months. Cleveland has no seasonal variation. The meals figure breaks down as $20 breakfast, $22 lunch, $33 dinner and $5 incidentals, with $60 on the first and last day of travel.

Agencies commonly benchmark stipends to these numbers. That is a market convention rather than a rule. GSA sets a cap on federal travel reimbursement and says nothing about private staffing pay.

The IRS rules on tax homes are the other half. Publication 463 is the source, and the sentence that matters most for a 13-week assignment that keeps extending is this one:

Generally, a temporary assignment in a single location is one that is realistically expected to last (and does in fact last) for 1 year or less.

And the one that catches people who keep renewing:

A series of assignments to the same location, all for short periods but that together cover a long period, may be considered an indefinite assignment.

Once an assignment is indefinite, the IRS position is that the location becomes your tax home, and payments for living expenses are income even when they are called travel allowances.

We are reporting what the IRS publishes and nothing more. How any of it applies to your situation is a question for a tax professional, and it is worth asking before you sign an extension rather than after.

If you are arriving for a Cleveland winter

Snowfall normals from the National Weather Service, measured 1991 to 2020:

Normal annual snowfall is 63.8 inches. January is the heaviest month by a clear margin.

Read that together with the parking rule. If your car is in an assigned garage you are covered. If you are relying on the Red Line and finishing after midnight in January, you want that worked out in October.

Where you are actually working

Cleveland Clinic operates main campus and 15 regional hospitals in Ohio, with 5,050 beds. Across the whole organization there are 23 hospitals and 17,600 nurses, though that figure covers Florida, Nevada, Abu Dhabi, London and Toronto as well as Ohio.

University Hospitals runs 21 hospitals and more than 50 health centers across 16 counties in northern Ohio, with over 30,000 staff. UH Cleveland Medical Center alone reports more than 3,515 registered nurses, 83% of them holding a BSN.

What we could not verify

• Processing times for any Ohio Board of Nursing application. Not published anywhere on the board's site. The 180-day temporary permit is the only hard timing figure.
• Any staff parking shuttle. The current Parking Directive contains no shuttle provision at all, and no schedule, route or hours are published. The only shuttle Cleveland Clinic publishes is for patients and visitors.
• Current staff parking rates. The public sheet is dated February 2021.
• A typical first snow date for Cleveland. Other National Weather Service offices publish one. Cleveland does not. The monthly normals above are the defensible figures.

A place to sleep between shifts

Thirteen weeks is long enough that the room matters. It needs a kitchen, because eating out around three twelves gets old in the second week and expensive by the fourth. It needs in-unit laundry, because scrubs. It needs to be quiet during the day if you are working nights, which rules out a lot of hotels.

What is the per diem rate for Cleveland?

The federal GSA rate for Cleveland and Cuyahoga County is $167 per night for lodging and $80 for meals and incidentals in fiscal year 2027, which starts October 1, 2026. It was $159 and $80 in fiscal 2026. These are federal reimbursement caps, not a standard that agencies are required to follow.
